Name Change Application 護照更名

For Australian senior citizens aged 80 and above who live outside Australia, you may need to complete a Proof of Life Certificate form to continue receiving your pension.

適用於年滿 80 歲且居住在澳洲境外的澳洲年長公民，您可能需要填寫"Proof of Life Certificate"以繼續領取您的年金。

The Australian Office can witness signatures on Notice of Intended Marriage forms.

What to bring:
- Valid proof of identity (passport or Taiwanese ID)

Group Lodgement - 2 Applications: Adult and/or Child 兩位護照申請 (成人/孩童)

An affidavit is a written statement that allows the person making it to present evidence in court or other legal proceedings. The person making the affidavit must take an oath or make an affirmation that the contents of the affidavit are true and correct.

What to bring: 
-Your completed UNSIGNED Affidavit
-Instructions from your solicitor (if applicable)
-Proof of identity

宣誓書: 經陳訴者宣誓在法律上可採作證據的書面陳述; 陳訴者可依個人信仰而採取宗教式誓言或非宗教式誓詞或聲明來做宣誓.

Affidavits witnessing宣誓書 見簽

Choose this service for all other documents requiring a witness's signature. Do not use this service for land transfers or witnessing signatures under the Oaths Act. Clients wishing to have their signature witnessed under the Oaths Act should select the 'State and Territory statutory declarations' service. Maximum five signatures per appointment.

Witnessing signatures (for non-property-related documents) 簽名見證 (不包含土地相關的文件簽名以及任何在Oaths Act條款下的簽名見證)

For the Australian Office to witness your land transfer, property or mortgage document you must bring along:

-Written instructions from one of the following: Australian legal practitioner, law practice licensed conveyancer, mortgagee (i.e. your bank)
-Proof of identity
-Correctly prepared forms

We cannot complete Verification of Identity or Witness Certificates from your bank or state regulatory body. The Office can instead provide certification or a DFAT Identity Certificate.

若您的澳洲土地/房屋買賣轉讓或貸款文件需由本處領事人員見證簽名, 請備妥並攜帶下列文件:

-由澳洲律師事務所,委任律師或銀行出具的具體說明信函
-身分證明
-正確且尚未簽署的文件

本處僅填寫由DFAT提供的"Identity Certificate.
恕無法填寫或簽署各銀行或各州政府使用的 "Verification of Identity" 或 "Witness Certificates".

Maximum 5 documents per appointment. This service is provided by our officers using the prescribed wording: 'This is a true copy of the document presented to me.' We cannot provide alternative wording. 

We can certify: :
-Australian documents 
-Foreign documents for use in Australia 

We cannot certify:
-Documents for visa and migration purposes
-Foreign documents for use outside of Australia

What to bring:
-Your document for certification
-Evidence of why the certified documents are required
-Proof of identity

The Australian Office can witness signatures on driver's licence renewal forms. The Office does not review driver's licences nor provide the relevant forms.

-Photo (if required)
-Proof of identity
